Does my SER5 (AMD 5800H) support a monitor with 5120 x 2160 resolution?

I'm considering buying a new monitor. It has a resolution of 5120 x 2160 at 70 Hz.

Does the video output ports on the SER5 support that resolution? My SER5 has an HDMI and a Display Port output. What's the version of each port?
